It all began with Ai, he was human he lived in Hole. He wished to become a magic user so he stuffed himself with the flesh of various dead magic users but apparently died after the operation. The operation was actually successful but caused him to develop a split personality and thus Kai was born, he is the cross-eyes boss. Kai wished to become even stronger and started hunting magic users and implanting their devil tumors in his brain, his endgame is to become a Devil or something even greater. After En began to look for him Kai had to become more cautious with his kills so he created Aikawa as a third personality, which doesn't know anything, and Kai began killing the people that befriended Aikawa. When Kai killed Risu he was cursed by his magic, in that very same moment a bottle of Ebisu's smoke Kai was carrying broke and as a result of the two things Kaiman was born, he's basically an amnesiac Aikawa with a lizard face.

Ai didn't die but Kai took over the body, Ai still exist within Kai. As for the multiple heads and bodies, don't get that and the personalities mixed up. Because Ai implanted in himself the flesh of eight dead magic users he gained eight extra bodies, whenever he loses a limbs or something he can replace it with one of the spare parts that are stored inside him. There is no correlation between the personalities and the heads. Kai created deliberately Aikawa for the reason I explained before, when Aikawa realizes what's going on he tries to kill and at this point Kai comes out and explains that Aikawa is just something he created and thus has no real control over the body. Kai lose the control over the body after the got cursed by both Risu and Ebisus's magic, that's why for some time Kaiman was the only one in command of the body.

For some reason that looks unappealing to me and I'm not sure why, I used to think that it might be because of those shit cleaning jobs that over use topaz and everything looks super smooth but that's not the case and this image looks like decent quality.
My other theory is that it's certain screen tones that they use (some mangaka) that look like shit to me but I don't know much about screen tones to be sure, maybe that's what makes this style look really smooth or something, while I prefer a more rough or messy style.

Can't really think of any others that have a huge deviation from the norm. I mean I guess you can say something like Jojo has a unique art style but if you read a lot of manga it's pretty easy to recognize different artists unless their drawing is generic as fuck.
Magician is a manhwa but the art really stands out to me. Originally the outlines for everything were rough like the artist drew them several times. Then around 40 chapters in the outlines completely disappear which gives it a very interesting look. They come back later and very clean. Plus it's full color and just generally looks nice. Pic related shows the progression.
