I remember once during the Buu saga Goku hurt his leg because he tripped over a rock in the dark.
Piccolo also put a hole in his chest with a laser and Krillin hurt his head with a rock. Goku being vulnerable when his guard is down seems pretty consistent to me.

Not the same situation. Yamcha genuinely lowered his guard and let himself be caught by surprise by an opponent who lacked skill but was just as powerful as him, so he couldn't escape from the death grip even if he pushed his ki to the max.
Meanwhile, Goku not only lowered his guard but also lowered his ki to the point that saiyan saga Yamcha could punch him, knock him out and steal his lunch
