Another one from ef, although it's from the VN as well. Chihiro's story is significantly changed in the VN and is a lot less dramatic than the anime, since it didn't have to deal with time constraints.Instead of ripping up her diary, she spends the night in the library talking with Renji. Just when she is about to forget about their being lovers in the morning, she gives it to Renji. Having Renji choose the option to rip it up results in a game over, while if you choose the option to not forget about her, they eventually get back together again at the end of the next heroine's story.

The manga has a longer timeline but everyone seems to be more messed up. And I don't like the ending.

You're kidding, right? Drugs were present in the source material, the novel. Satou order the drugs via online, if I remember correctly. It's not explicity stated beyond it being called a legal drug, yet it's implied it's hallucinates or a drug of that sort. And the manga added and build upon the novel too.
In the anime, they opted not to show Satou indulging in drugs, for reasons that should be obvious.
