Orgasm denial is all about building tension. In terms of story structure, you need to have a release of some sort. You can't just keep building tension indefinitely. Eventually the effect diminishes to the point where you're not even blueballing the reader because they just don't care anymore. You can use it as a device in the short-term as long as you don't abuse it, just like any other story device.
