She was doing her very best at work, so that she could feed her daughter. She's a lone mom, with a daughter at 3, but she still have to work overtime at the low-paying secretary-job she has. Her boss probably had a meeting, and it stretched into the night, and her daughter was at home alone after the baby-watch left at 10pm. Daughter didnt eat at all after dinner, and as it gets closer to 4am, she was crying in the apartment, for her mom to come home. When the mother finally came home, she had to feed her daughter, clean up the mess she had made during the hours she were alone, and then she went to take a shower. Since the dad ran off as soon as he heard she was pregnant, no one compliments her anymore, her parents doesnt want to see her anymore, the neighbours dislike her because her daughter cries all the time, and her boss gives her a hard time at work, demaning her precense at inhuman hours. So when she finally goes to bed at a little past 5am, the fever hits, and she feels so bad, that she's laughing at her own misery in the bed, her entire body burning up, mind numb, and she doesnt really feel the will to live anymore.
That's what the face conveys.
