You forget she was up against QB, who not only has more resources and knowledge of the system than her but is also completely immortal and seemingly omnipresent.
Add to that the fact each loop caused Madoka to become a bigger and bigger target for QB, and you have the recipe for a cycle of failure.

Things she actually had to do.-
1-Make a record of major events in the timeline.
1.1-Make a copy on your closest stationery for every girl to have, the instant they see a detail that was already written on said record, they would at least be more willing to listen to Homura.
2-Only go to school ONCE, spy Madoka from afar.
3.-Kyubey was just about to leave Madoka and Sayaka on the 100th loop. Homura just had to stop Sayaka and nothing would have happened.
3.1-Threaten Sayaka to use her wish.
Have her family or Kyusuke hostage until she wishes that Madoka is not able to become a magical girl or just be cool and ask her to wish that Walpurgis doesn´t appear at all.
3.2-Don´t be a fucking bitch to Mami or Kyoko, have them join your side to defeat Walpurgis.
Or at least keep them from killing themselves with point 1´s record.
4.-Use the fucking Grief seeds you KNOW that will appear, it´s the 100th time she has done this, right?
Make a fucking list or remember when and where they would appear.
5.-Either team up with Mami, Sayaka and Kyoko without being a fucking edgy bitch or make Sayaka wish to make Madoka unable to become a witch when she contracts.

Things she did differently-
1.-Defeated Charlotte Early.
2.-She was kind and a pal to everyone else.
3.-She actually told Mami about what happens to Magical Girls.
It is different from the anime since, she just discovered from having to kill Sayaka.
4.-Sayaka still contracted, but didn´t conflict with Homura and learned that she meant well for everyone with her spoilers.
5.-She was in her "nerd" persona, so everyone cared for her.

Mami discovered what happened to Magical Girls just after having to kill Sayaka, so the trauma is even greater than what happened in the PSP game, where Homura just told her CALMLY.

The PSP game does that, actually.
There is a happier ending where everyone survives and are best pals and other endings where everything ACTUALLY goes to shit.
Every end exept the best one and the canon ones end with Homura retrying.
The 'problem' is the story is designed so that there is no real way to achieve a perfect ending because girl drama. The anime depicted the closest thing to a perfect ending with Madoka dying and everyone else surviving, and Homura wouldn't accept that. It drove her insane.

For someone that is supposed to be obsessed with Madoka´s future, she shouldn´t have wasted that much time on her house as she did.
What i am saying is, if you have the ability to travel back in time, the obvious frist step would be to spoil exactly what will happen in the future to everyone with details, so when they see the event happen in the exact way you described it to be they would be at least willing to listen to you.
Or just kill Charlotte Early and have everyone pointing at Mami when you tell her the truth.
