Realistic fighting is really more beneficial from a plot standpoint, all it does for the fights themselves is limit the potential for the choreography. I guess you could say it makes them more brutal or that it raises the tension a bit, but there's other ways to do that.

The most realistic I can remember is Shigurui. Some of the stances and grip techinques can seem retarded, but details like the strongest characters being all muscular as fuck, the short length of the sword fights and the disgusting gorey details gave a realistic feel to it.

If I were an anime director, I'd hire a team of expert swordsmen of various styles to work out the choreography. I'd also hire a bunch of /fit/ physicians to handle the movements of the superpowered little girls. Of course I'd do all this in secret and forbid them from going public.
The battles would be short and uninteresting to look at and the girls would look like they're spastic or something. Everybody would hate the action aspects of my anime until one random anon mentions that it kind of looks realistic only to be shit on by everybody else for liking crap.

Studio 4C did hire someone to help with the fight choreography for the Berserk movies. However, fights in forms of media are hardly ever made to be fully realistic with good reason- it would be relatively boring to watch. A good fight doesn't have to be realistic, it just needs to be believable.
