You'd need to have AHA/EMT cert'd volunteers registered through the app as first responders that the local EMS lines could call upon.
So it'd still need to drift back to governmental management via the NHTSA (in the US at least).
Then the local governments would have to step in and make sure those registered were protected under the law for helping but things go bad anyways.
