Homura's wish was to protect Madoka. And she made that wish, you will note, before Madoka asked Homura to save her in timeline 3. Madoka never asked to be brought back to life from fighting Walpurgis Night; in fact, she said she had no regrets. It was Homura's decision to undo Madoka's sacrifice, whether or not Madoka wants her to, and that's what she's been doing ever since.

Yes. Rebellion showed how selfish she really was. In the series there was a greater evil out there and a cruel fate for everyone. Madoka fixed all of that, and Homura couldn't let go until she ruined everything. She didn't care doing it.
