Ok this was a joke but it sounds delightfully gross. It WAS aliving dungeon, but it's disgusting and rotted, and festering with giant decomppsers like worms and stuff. Plus some zo.bies or whatever of other lost travelers.

The dungeon is inside yourself. You are literally walking inside of your own body. If you do too much damage to the surrounding walls, you will actually hurt yourself.

If you also take too much damage from monsters inside the body dungeon, you might actually risk drowning from the blood as wounds open up in the walls. And, if you end up in your own lungs, and you start drowning in blood, your lungs will start filling up with more blood, so you wind up drowning in blood and filling your lungs with more blood and drowning in that more blood.

It's not a discrete, individual organism, or at least you gradually encounter more than enough major organs for a dozen absurdly huge monsters and can develop no plausible idea for how this thing's body is laid out. In general it's one of those megadungeons that goes arbitrarily superdeep.

It's not a living dungeon. The doorway was just a portal to the Plane of Meat.

It's a giant stone ooze. That is, it is a living dungeon, but still made out of rock.

The entire dungeon is under the effects of a Time Stop spell, and so while it all looks like meat, it's immobile and hard as stone. (And all starts falling apart as soon as the Time Stop ends, such as beating the dungeon.)

You Are The Golems. Yes, it's a dungeon and its alive and its made out of meat. But the "people" of the world are spirit entities and are the ones truly alive. Being shackled to a physical body is no more living than being a plant. Bonus points if you can get your players to be wandering spirits all along without them somehow realizing it.

The dungeon is actually a massive, petrified creature, sculpted into something livable, but the magic is failing, or perhaps the party needs to take whatever keeps the effect up. Despite the sculpting, the creature will survive the return to flesh, at least long enough to be an issue for the party, and perhaps end up as an undead dungeon after that.

It's also been done. A hefty chunk of the Guardians of the Galaxy movie took place in a giant corpse, for example.

The dungeon is the corpse of the god of good stuff. You have to revive the body with macguffins and feelings before BBEGod takes over. The world outside is wholly evil now, you are the only force of good left. Outsiders keep cutting into the god's body to hunt you down, so you have to use various organs and glands to create deadly traps to slow your pursuers while reviving each bodily system in a proper order.

Parasites are everywhere, upgrades are from corpses of other fallen good guys and enemy bodies strewn throughout the cavities, and the god is about to give birth.

In a modern realistic setting, a gigantic cadavre of creature is found washed on the shore of some place, and you have to enter it for health and security (and secrets) reason. Cyclopean forensics

Or a guy turned himself into a dungeon by surgery and bio-mechanics, and then invites your party inside of him. Think Mother 2's dungeon man meets taxidermy

Reminds me of a setting my friend ran for us that was pretty gross but ended up being fun. Basic a typical D&D world was devoured by a huge space worm and we were playing as the descendents of (or in the case of long living races sometimes as) survivors of the cataclysm who reached safety through planar gates. Some of the gates survived and a camp had sprung up around one such gate which was used as a base for explorers and adventurers hoping to salvage whatever was left inside the worm as it slowly digested the entire crumbling planet. Whole bunch of homebrewed monsters themed around the ecology of the worm's insides. Myconids were a prominent race as some of the spores survived the event and had flourished in the new warm damp and dark environment. Other gates survived that lead to less appealing destinations so you've still got some classic D&D monsters spilling in through them. "Tremors" as the worm continued to break up the bigger chunks of the planet were a major environmental hazzard as were gasses so fire was a bit of a no-no and we had to provide our own oxygen at times. Shit was crazy. Essentially we were intestinal parasites trying to reclaim resources the worm needed. Culminated with us killing off the worm's offspring before they could be born, saving countless worlds. Worm ate some bad food.

The lich inside it now is using dead soul juice to create a undead titan. The dead soul juice is also the catalyst enhancer for the steampunk civilization.
