I think it was like you had to select your head, not your mouth, which everyone did.
And then it threw people off because it'd say "you stab x in the head" instead of "you stab x in the mouth".
But yeah we both got it to work after I woke back upand then the joke was ruined because mezzo went and told everyone how to actually do it then went and fixed it so I was left with a bunch of useless omelets.

Well, there's not much tension to most horror rounds. Because the monsters either aren't super scary, or just straight annoying. Mannequins for example are a joke if you can make it to the armory and throw on some armor then they just become irritating bullet sponges, even then their not super scary because you can't run from them even if you can't arm up. Most other things are horde mobs they instantly stop attacking you the moment you break LoS and don't have much of a lasting effect even if they do hurt you.

most people don't react the same way to it anymore despite it still being extremely dangerous.
veteran players just have a harder time being scared of things, a good example of this would be the left 4 dead series where people hump witches and tanks with their shotguns despite their first experiences being far different than that.

Fear is founded in uncertainty. NPCs don't stay scary, because they quickly become understood. PCs can stay scary because you don't know what the fuck they're going to do. Overt admin intervention can make something go from scary to not scary both because it breaks immersion and because "shitmin with buttons" is a known quantity.
Good horror is emergent and nearly impossible to force unless the players want to pretend to be scared or there's a fancy new bit of code.
