Have a bloodborne random roll weapon table, roll 2d10:
>Covenant:
1-3 "Hunter's -" (workshop): Adds half of its damage (rounded down) again to any attacks vs. targets classed as Beast or Kin
4-6 "Holy -" (Church): Transformation causes all damage dealt by this weapon to be treated as Arcane as well as physical for the purpose of armor penetration
7 "Martyr's -" (Executioner): Transformation allows user to give themselves upto 4 damage to their torso in exchange for an equal amount of arcane damage to target.
8 "Knight's -" (Vileblood): Transformed state causes user's torso to take 1 shock damage every turn in exchange for adding +2 blood damage to successful attacks.
9 "Explosive -" (powderkeg): Transformed state allows user to perform a special attack that slows the attack by 4 but multiplies damage by 2, with additional damage treated as fire damage.
10 "Beast's -" (Church): +1 killing damage and effect identical to taking a Beast Pellet while weilded in its transformed state
>Transformation:
1-2 "- Long -": Adds 5 feet to range at which it can strike enemies. Additionally by reducing the speed of your attack by 3 you may make an attack roll against everyone within 5ft of you.
3-4 "- Giant -": Permanently reduces the speed of your attacks by 1 as long as it's transformed. But every time you successfully hit an opponent you knock off two dice from their set rather than one.
5 "- Shocking- ": Half again (rounded down) additional Bolt damage
6 "- Burning -": Half again (rounded down) additional fire damage
7 "- Uncanny -": Half again (rounded down) additional Arcane damage
8-9 "- Merciful -": Splits into two smaller weapons (requiring both hands to use) and +2 faster in transformed mode
10 "- Rifled -": Adds an inbuilt gun that does half the weapon's damage (rounded down) to targets out to medium range at the cost of 1 quicksilver or blood bullet per shot.
Then finish the weapon (and its name) off with a base weapon (like Sword, Wheel, etc...)
