Depends. In the case of Living Saints you have to die, come back, and win a losing battle. Being a priest or sister also helps. Lastly, the Inquisition and Ecclesiarchy have to agree to give you the title. That probably means any psykers trying to pull a fast one get caught in case evaluation.

Well yeah, they're more devoted to the Emperor than anyone else, so it's pretty believable that their psyker abilities are a lot more focused and "dependable", which is what miracles are; psyker abilities without the chance of their head popping and farting out warp spawn or worse. Hell, they literally bring their best and brightest back to life as colossal angels with flaming swords just by asking for it. Shit like this is why I don't know why the SoB aren't more popular, they're more dedicated than Spess Mreens and have pretty much the same armor, just no gene seed or genetic tomfoolery.
