Chaos worshipers start to resemble their chosen God or it's daemons as they start gaining various mutation gifts. Nurgle's daemons tend to be bloated and rotting, with bursting stomachs and single cyclopean eyes or three in a triangle. His devout followers also begin to gain these traits, even if they don't look exactly the same as Plague-bearers or Great Unclean Ones. Meanwhile Slaanesh and it's daemons tend to be distinctly feminine/androgynous and it's pretty common to see them sporting one, if not more, breasts. It isn't a great stretch of logic that Slaaneshi worshipers tend to start growing tits along with their giant crab claws.
