I've gone through something that tight before, but it wasn't a plain hole in the ground.
It was a funnel 2m at the top, shrinking down to that size then going horizontally for 5m and then a big round cave.
I went in with just my pants and a frontal light on because I was wearing a good shirt, I knew it was safe beforehand but it was quite stressful.

What about a normal city that happens to be also a dungeon?
Some doors, any door, maybe the one that leads to the kitchen of the tavern, or the bathroom door in a house, leads to a certain part of a dungeon.
And some entrances and holes in the dungeon lead to parts of the town.
Let's say.. you get into the bakery just to end up in a room with some monsters. You kill them and you go out. You explore the place a few moments but when you enter into another room of the dungeon, you end up appearing in the tavern, or maybe in the bakery itself.

Maybe the whole city is an illusion.
Maybe the portals that lead the players to the dungeon don't affect the citizens.
Maybe it's all a huge conspiracy.
Maybe the citizens aren't what they seem to be.
Maybe all the citizens are already controlled by the boss of the dungeon, so the monsters under his service don't attack them.

Hell it's a damn game and it probably involves magic or high-tech, you can fill the plot holes in any way you want.

Most of the ideas I mentioned sound fine to me... hmmm
Probably the most appealing is one where the citizens aren't aware at all of this. So maybe they are not affected or they are somehow hypnotized or something to not detect the dungeon parts or the monsters.
That might lead to interesting situations where the party, trying to save everyone, end up getting in trouble in the city cause they are acting like mad men.

It's a drainage well in the pond. You can see the raised edges of the well under water. Since it's slightly below the water level, water keeps flowing into it, and presumably through drainage pipes into a river or something.
Judging from the grass growing on the inside, the water level on the pond varies between seasons. The well is presumably built to prevent flooding during the time of the years the water level is at the highest.

My favorite dungeon entrances are innocuous doorways in completely mundane palces. I once ran a game where they party went through like 8 session dungeon whose entrance was at the back of a pantry of this random peasants house that the PC's were helping out. There was a rusty padlock on it that the rouge easily got through and a sign that said "keep out! Rats!" but, ya know, PC's cant read signs.
