Matriarchal monotheistic theocracy loosely based on the 7 Noahide laws. Interpretations and later additions have been the basis for a hundred differnet religious order all scheming for dominance. Due to an ancient agreement the military is mostly made up of orcs from the coastal cities of their mostly desert nation.

Traditionally the Sun Empress has to be married to the Warrior-King of the orcs.
This also makes the whole hierarchy shuffle around drastically as orcs and elves can't produce children together so every succession is uncertain. Civil wars are not all that common as the Orcs make up almost all the army and a won't use weapons on each other. They prefer to make settle disputes in a fighting put with bare knuckle boxing.
The remnants/results of the previous empire are still out there wandering the sands. They were a cruel and vicious society. They made pacts with devils and spawned from forbidden unions a race of tieflings.
The tieflings are quite happy as nomads and wandering traders. Every so often one of the pre-exile nutters or idiot youth tries to rally everyone with the dream of taking back their homeland. The more sensible elders take no heed of them.

Take Tibetan monks and mix them with a completely different culture, like the Vikings. Make sure to use loose analogies of both cultures - to go particularly in depth makes it harder to mix them unless you stretch.

I actually did this in a homebrew I was GMing based on the saga of the founding of iceland. The PCs were living a Norse lifestyle, but of course there were elves and trolls and to make them more alien and terrifying I had them follow a Mesoamerican aesthetic, sacrifices and all.
To keep the PCs from getting the hint right away, I described the Mesoamerican crops as one would if you were a Viking who had never seen them before. Chilis were long wax-berries that cause incredible pain to consume. Cacao pods were bitter-beans and so forth. One PC didn't realize what it was until he tried to throw a "pain-berry" at the great aurora bird-wyrm (an avatar of Quetzalcoatl), at which point he stopped, realized what was going on, and that he tried to defeat what is essentially a dragon by throwing a chili pepper at it.
(He rolled a 1: the entire encounter ended badly. As an aside, he continually rolled 1's and was known to be a terrible fighter, except for the one time he defeated an 8 year old girl in an honor duel.)

My first instinct was a woefully un-specific wish made by the elf elders to a sardonic and uncaring god in exchange for the immortality of their race or something along those line.
