Pakistan is a shithole - it's exponentially growing population and culture is stupid, rapey incestuous, corrupt and fanatically Muslim. It's much worse than India and the Taliban have re-located most of it's forces into the vicinity of Pakistan. They emigrate en masse to Europe and are met with little to no resistance. It is a haven for terrorism, and a majority of the population are ISIS collaborators. Also, Pakistanis are basically Muslim Indians (Indians being a brown, unhygienic yet friendly people), so just recognizing that will help you understand how just much of a threat Islam is to the Western world. Why doesn't the United States just eliminate Pakistan?
tl;dr Pakistan is a largely unrecognized source of evil in the world. Why don't we extinguish it?

Can someone explain why they have a "minimum" requirement to get a subsidy when typically you would think they would only have a maximum as yeah those people have more money so they can (in theory) afford the higher cost but the lower you make the less you can afford the cost

It's a States Rights issue. See, the ACA was developed to work WITH the states, not FORCE the states to do anything. It works best when your state has expanded medicare. The Federal government ponies up some taxpayer money to cover the State's cost of doing so.
But if your state said NO to this expansion, there is no coverage. See, a state decides where to apply medicare. So, for instance, in Florida, if I make more than $4,000 a year, I am NOT ELIGIBLE for medicare. The Federal government went to Florida and said "If you raise your upper limit to $12,000/yr, we can cover all of these people. In fact, we'll PAY you to do it, 100% of the cost, for 5 years. After that, you pay 10% of it."
Many states said no. They were opposed from the beginning to the ACA.
