Well, first off you've got to get them. You can do that either online, or by going to a therapist who will refer you to an endocrinologist. Going through a therapist is probably the better choice, because the endo will help watch your hormone levels so you don't hurt yourself. Informed consent is also an option if you can find a clinic near you. IC basically just skips the therapy step.
Pros (for femboys):
skin softens, body fat redirects to more feminine places, changes to feminize your face, less body hair, no chance to further masculinize, hairline can restore and won't ever recede further, etc...
Cons:
penis and testicles will shrink and you will become infertile, you will stop producing sperm and won't ejaculate during orgasm, you might have a hard time getting an erection, you will probably have a bit of gynecomastia, the hormones can really damage your liver so you might have to get an orchi at some point so you don't have to take AAs, etc...

Taking mones does not make you a tranny.
Hormones can help you feminize if that's what you want. An estrogen supplement (estradiol, estrace, etc), an anti-androgen (Spiro or Cypro), and sometimes medroxyprogesterone are the most common hormones.
You also don't have to take any drugs at all to feminize. If you're fine with masculinizing later in life. Just take care of your hair, skin, body size, and everything else basically religiously and you might have a chance at that. That will only work if your genes are pretty much princess tier already, though.

Nope. You'll basically go to see a therapist, and tell them how you feel. They're basically there to help you better understand what you're feeling and to make sure that you're ready to do something as drastic as taking hormones. You might have to see them quite a bit before they'll sign off on it, but eventually they will write a referral for you to see an endocrinologist. The endo will be able to prescribe your hormones and also tests your hormone balances every 6 months or so to make sure they don't get out of whack.
