Don't have any experience with Quake 4, but I'd say something's wrong. I think your integrated graphics should be better than that.

I fixed it since making this thread.
It couldn't detect my refresh rate and setting it manually has allowed me to achieve 60 for at 4k resolution.

Thanks for any concerns, gonna play this game I missed out on now.
