6950 2gb
16gb ram
Can i just hot swap GPU for something newer and leave the 920 as is or do i just go for all new build?
I only wish to game at 1080p.

CPU may be a bottleneck, but it's still the equivalent of a budget AMD processor.
You could start looking for an i5 2500k or i7 2600k package on whatever passes for your Craigslist.

If you can OC a bit higher the CPU may be on for a little while longer. Start with a new GPU first, id wait for the RX 480 of you intend to stay at 1080p.
If the cpu is holding you back after the above, then look into upgrading the rest.
