take a look at the event viewer, there might be some errors.
I had same problem, turned out controller of the SSD was faulty

It could be your SATA controller. Do you have another SSD you could try to test on the same SATA port to make sure the problem is on your SSD?

Is it an x58 chipset? Many older motherboards have a marvel controller that cannot properly support advertised SSD speeds even with AHCI enabled. Pic related.
