If it's a FAT partition it might be that the file allocation table is pointing to the root weirdly (the root directory in FAT is always statically defined when a device is formatted).

FAT corruption.
Solution is not to use FAT or remove the drive while writing.
Recovery software cannot fix it, because that part of the filesystem is overwritten with corrupt data the next time the drive is used. Can happen with NTFS in some circumstances too, but that completely blocks the drive from reading by confusing the MBR of an ejected drive for the MBR of the current one and creating completely unreadable garbage (though that IS recoverable).
