This is my old ass PC, I want to change the GPU but the motherboard is outdated, not to mention it's only a dual core CPU. What do you guys think I can get that is substantially better then my current GPU but is still worth getting with that CPU?

That's an AM2+ board and supports Phenom II chips. An X4 945/955/960T/965 shouldn't cost much at all on eBay. Get one of those and you could upgrade to a 960 without any trouble. It's not worth buying anything at all with your current CPU. It's literal garbage at this point.
