Karnak is called an Inhuman despite never having been exposed to the mists.
He's just a human/Kree hybrid. So the inclusion of him in the Inhuman's ranks means that the mists are not a defining element of the definition.
The culture he's a part of can not define what it is to be Inhuman either because innumerable people worldwide who were unaware of their Kree lineage have been accepted as Inhuman once hit by the mists.
