It's called 3D Custom Girl, and is an old-as-hell hentai game. It's basically just a little game where you can make a girl, dress her up, and fuck her. However, there are like 8 billion mods for this thing (diapers being one of them), and the devs later added an option for posing your characters so you can make images. I do not know how to post yet, but if people like this stuff I might get my ass in gear and learn.
I should warn you that there are basically only two diaper mods--disposable and cloth (I'll boot up the game and show you some pictures of the cloth diaper in a bit). Both are somewhat flawed but good enough---3DCG really isn't meant for underwear thicker than panties. Also, said mods look off on loli characters. We got an modellers here willing to make some mods?
There aren't many other ABDL-friendly mods, there's a pacifier which I love and a baby bonnet which is nice.

Be warned that this game is pure jank and stuff can look weird. For instance, you can't have your character wear pants and a diaper at the same time, because the diaper will show through the pants and look awful. Also, the disposeable diaper mod is in the tail slot for some insane reason, I modified it to be in the underwear slot and can give you the mod file if you want.
There's a fanwiki but 4chan thinks the link is spam. Just google "3D Custom Girl Wiki" and pick the one that ends in .tm
There's this torrent that has the game and FORTY GIGABYTES of mods (and each mod is like a megabyte only so that is a FUCKLOAD of mods). That's not even all the mods out yet! You'll want to use the mod catalog, which is kept up-to-date nearly every week, to search out for what you want. I'll upload the torrent in a second if you want.
Just remember that it's not a game meant for diapers. You can just use some diaper mods. Keep that in mind and you can have fun.

I tweaked a few mods: I made the disposable diaper mod an underwear item so I could use tails with it (otherwise that demon girl above wouldn't be possible) an I changed the pacifier to go in the mole/facial mark slot so I could combine it with another mod that adds drool and a heavy blush to your girl.
Unfortunately they're all in Japanese and I have like 200 mods installed. So I just shoved every diaper mod I know I have, so one of them should be the modified ones. Just put these all in there. You know it's right if the slot with underwear has the diapers and the slot with the moles has the pacifier.

That's a good idea to ask your therapist first. If you can get on through them somehow or get it covered by insurance somehow that would be a lot better. Even just getting one temporarily to try would defiantly be a good idea if you are able to before buying one. The price and the inability to try one first held me back from getting one for a while.
