That was the "Rho Aias" that Archer made to help Shirou in a way that wouldn't let Gilgamesh know there was someone else waiting to ambush him.

Irisviel is dead, but the grail copied her personality. If we are to believe the grail copied Iri to perfection, yes, she'd hate Kerry for killing Illya. However, as that was an illusion and not real, we can't know how she'd react to the whole thing, even knowing it was fake.
Also, the grail copy of Irisviel personality might have become corrupted with time. It explains why it looks like her, talks like her, but all in a Silent Hill sort of way.

He only died 12 times in the VN, but the 12th time he died he did not evaporate into constituent magical energy. He instead did the impossible, shoved aside the laws of magic and the world, and remained in existence so as to be invincible for Illya.
But it's hard to convey that without a narrative. So instead, his death total got ramped to be more impressive in return. Killed 12 times, gets back up, is killed again, and THEN breaks the chains and lunges at Gilgamesh before being killed a 14th time.
