Mouse click on the red bar to determine power, then click again when the double crosshairs are as close together locked onto the dude's crotch as possible.
the more accurate your hits, the less the yellow bar at the bottom increases, and if you manage to get over a certain number of points before the bar fills out you get a special graphic of the dude, otherwise he just passes out in the same position.
Another thing is though is that the lower your power, the slower the crosshairs are in the second part - so you could choose to play it slowly. The method of assaulting his nuts varies at different power levels too.
low -> squeeze
mid -> foot grind
high -> punch
