I think it's less that he's selfish and more about the fact that his force of personality is just so overpowering that other people can't help but be drawn into his life and just do whatever he wants.
For him, it means that he's absolutely unable to have a meaningful relationship with someone, because he simply, and without any intention of doing so, twists them into a puppet to his whims. When it really comes down to it, Rick wants to be loved as an equal, same as anyone else does, but he's so isolated from the world that's something he can't manage.
In the entire universe, he is completely alone, and he can't seem to help but destroy everything he loves, no matter how hard he tries.

Having been in a relationship like that, that's not super accurate. You both know that you're horrible, so you let each other be horrible. To be honest I didn't like the NIN selection here. It was too on the nose and lacked any hint of subtlety.
