I guess Gargoyle's quest kinda counts. You're a gargoyle saving a bleak ghoul realm from stronger monsters.
I want to say Mana series should count from grimdark too. Not much for style, the games are colorful, but holy shit the story in these games is depressing as hell.
Vagrant Story definitely counts too.
Castlevania as well.

King's Field, Ghosts 'n Goblins, Gargoyle's Quest, ActRaiser 2 and Castlevania come to mind.

If you're also looking for games with a similar playstyle as Faxanadu, then Zelda II and Battle of Olympus, although they follow a Greco-Roman theme. Castlevania II is another consideration. I'd weakly suggest 8 Eye's and Trojan as well.

The Druid series is pretty cool. It's top-down view, sort of at an angle (hence the shadows).

Chakan! I loved that game, still kind of do despite its shortcomings. It's very gritty and grimdark, the music actually makes a strange good use of the Mega Drive's sound chip (it's typical US Genesis game tinny crappy sound, but it works in its favor, amusingly). Quite an underrated game, although some of the bosses can be rather bullshitty, specially the big worm thingy and the final boss.

I suggest Demon's Crest for the SNES. Blackthorne for the PC also comes to mind but that's not really medieval. More of a futuristic medieval mix (orcs with shotguns). Both great games though.

I forgot to mention Dungeon Master 2 earlier. That game has a very sweet dark feel to it and also features nice RPG elements. Stats are raised by simply using abilities (attacking with different weapons or unarmed, making potions and casting magic) and have certain tiers to them. Resting will make a character level up if he has trained enough in such a way. But almost more important is the equipment that you can either buy, find or receive via solving puzzles. Each piece has different stats and sometimes hidden abilites (like those delicious mithril socks). You can venture alone to fight the end boss or you can release up to three other characters from the freezing chamber to fight along your side. They all have their own charm, though some seem more useful than others initially.

Damn, now I feel like playing the game again... Never finished the boss anyway.

You can switch between buildings you "look" at left and right and theres like a tent and you can go into a dungeon and fuck shit up. Dragonview is one of my favorites. Definitely an under rated game. Navigating the world map kind of sucks but its got a nice combat system and dungeon crawling.

I'm not sure if spinoff describes it. Xanadu was a huge hit on computers and Hudson originally wanted to port that. During the development they simplified and streamlined it for Nintendo players and hardware limitations (e.g. lack of disk save) and ultimately made a completely different game with only the name left as a reminder of its origin.

Not quite grimdark, but maybe the Wizards & Warriors series. The first two games take place in exclusively dark environments, although the designs are kind of cartoony, and the plot isn't too serious.

The SNES version has nice graphics and music, along with more levels and a 90 minute time limit as opposed to the 60 minutes most versions give you. Unfortunately, being an early 90s title for a Nintendo system, they censored the gory deaths. It makes me wonder how they handled the blade traps that cut you in half.
