1. Isaac must die to a Mulliboom in one of the Basement floors. Mulliboom seed: FHRD S1N1 (Go into the large room above the starting room. You can easily die from full health in here to them.) Or if not: JTMD EZPF (one is directly below - Normal difficulty)
2. Magdalene must die to her own bomb in one of the Caves floors. (Pyro Seed: ALFA 0NTZ)
3. Judas must die to Mom's Foot as Judas in one of the Depths or Necropolis floors. Good Seed: 8LDE 8XDX (Wait on using the Joker till the 3rd floor and avoid the devil rooms and you'll get the sacred heart. You also get the Sad Onion on the first floor.
4. Azazel must die to Satan in Sheol (his blood/laser form or hooves, NOT leeches or The Fallen). You must NOT use a seed.

It's not even a debate. Keyboards have ghosting which will not let you move diagonally while moving a ludovico tear diagonally at the same time. I had to rearrange my controls to YGHJ and Num5123 to be able to do that. Then I found out I can't use actives when moving diagonally up-left. Was a problem with How To Jump.
Though my cheap piece of shit controller has different inputs on analog stick compared to the right-side buttons so I can't switch to analog at will when I get Ludovico because up and left are reversed and so are down and right.

Suicide King card is actually really useful, albeit rarely so. If you've got the extra life to spare, there's practically no reason not to. It is almost guaranteed to give you a decent chunk of HP and an item or two.

In "Binding of Isaac: Afterbirth" the direction of movement is analog -- meaning it's possible to have a very high rate of control over the direction that you are movingCompare: Pacman (digital movement: up/down/left/right) and Mario (analog movement: digital input + angles inbetween those standard inputs)
Because you have the possibility of high control over your movement via analog input, and because the relevance of your characters location matters to the gameplay (dodging, accuracy, etc) it would be beneficial to use an input device that allows analog input over digital.
This is in a vacuum, of course, and it assumes that the devices you use are equal in every other way, which isn't true.

4b. run out of bombs on Satan and lose because he's immune to kamikaze and you didn't get any familiars/orbitals

Posted last night when I beat Hush by literally standing next to it. Tried to do so again today. This time, I didn't have the D4, so I know what my items were, and once again during its continuum barrage, I never got hit. I didn't seek out any safe spots, I just stood still. I had nothing that could have deflected the shots. I didn't even have orbiting items.
I'm still gonna fuck around with it, but so far, it seems like not moving at all during the continuum phase means you won't get hit. Still gotta be careful of those tank flies, though.
No matter how hard you try to wrangle the retard, he always seems to move in exactly the wrong way for the room.

When you suicide in the room you release a ghost (The Lost) which makes sense thematically, even more-so when you add in the idea of the "Missing" Page. The player kills themselves to find the "missing ghost" which is obvious in retrospect.
Well, alright so what if it was thought of not as a "suicide" room, but as a "Sacrifice" room, and applied the logic of The Lost? Basically, trying to approach this from a designers perspective instead of working backwards as the player.
That's the hard part I guess. Throw me some idea's if you've got them.
My current ideas are;

So fun thing. I stopped playing a while back when Afterbirth was announced, figuring I was late to the part with getting Rebirth, I might as well wait until the DLC is here and play everything.
Dumb move, I know, I could have been unlocking things.
Afterbirth comes out and naturally I jump straight into Greed Mode.
Not pictured since I was too slow, but this wave was both of the Angels at the same time. Meanwhile, it's worth noting, this save has never even fought It Lives!, I still encounter Mom's Heart. I thought the Angels were unlock determined and you couldn't encounter them until you had unlocked them, but Greed doesn't seem to care, it threw both of them at me at once.
Not that it mattered with Ludovico on Azazel (was unlocking Lilith), but still. Kinda a surprise.

Really even without all that it's too easy with Lilith. Each wave counts as a room, and Box of Friends is a 2 room charge, so every 2 waves you get to use it. By the boss waves you got 5x your familiars.
Batteries, Nun's Habit, or anything else, is gravy.
