Blood sacrifice and arcane magic ritual to reach out to Mergo the Great One in search of knowledge and power, manifesting Mergo as The One Reborn, a giant gestalt body made out of sacrifices. Multiple Great Ones are shown to have existed- there's two Roms, multiple Amygdalas, Four Failures, and as noted by the artbook every giant tombstone in the game world is for a Great One. The Great Ones don't revive, there's just extras. There's two Ebritas- one the Church found and one still wandering about the labyrinth unseen, and so forth.
You should also take the Hamlet as a grain of salt, it's a memory-world created to hide the history of the place. Therefore we can say Kos's electric pulses may have just been dying spasms or maybe the Orphan's own power. Who knows?

There's billions of them in the "mining" area.

So apparently it was originally planned that the guns in Bloodborne would need to be manually reloaded -- the guns would have variable reload times that depended on their type. Super high-damaging guns like the Blunderbuss, Cannon, Evelyn, etc. were all muzzleloaders that would take a while to reload, while breechloaders like the pistol were parry tools that did little damage. Players would be able to move and dodge while reloading, but not attack. I dunno how they would have managed different ammo types.
Would you play a BB with manual reload, different reload speeds, and different ammo types, /v/?
