Sonic 2. My older brother played it a lot, so I watched him. I think when I was around 3 he began showing me how to actually play it, along with the other genesis games we had.
This has always made me wonder, though. Some people are naturally good at videogames while others, no matter how hard they try, wind up being mediocre at best. Does starting out earlier give you an advantage of sorts, allowing your mind to adapt to the challenges more easily during such a critical time of growth? Maybe I'm just overthinking this.

Back in 1991 my dad came home from a garage sale with some kind of atarti. It had Qbert, which I could never play well, Asteroids, A truck driving game with a special controller for changing gears that I could never figure out, and a froger like game with a chicken crossing the road, that one was my favorite. I would wake up a 5am when my dad left for work and get him to switch the TV coaxial cable from the TV antenna to the atari and play until my mom got up and watch to watch the CBC news on the antenna.
A couple years later I got a saved all my Bday and xams money and bought a sega genesis that came with Sonic 2 and the Jurassic Park game. These were the first two games that were all mine.

I almost reflexively said "Sonic the Hedgehog" but then I remember I only watched it being played by my Dad when we lived with my Grandma. I probably did play it, but I have no memories of it so in the spirit of the question, I'm not putting it up as my answer.
My after-school club had a Super Nintendo and I sank all my time there into Super Mario World and Super Mario Kart. My babysitter before I went there had a Mega Drive, and her son had Aladdin and Ecco. She also had an Amstrad GX4000 and only one game for it (guess which one). But I definitely played a videogame before I even had a baby-sitter and it would've been on the Mega Drive. I played Sonic 2 non-stop whenever I was home until I got a Playstation in my room (then it was Rayman and whatever game I borrowed from a friend. Crash 3 saw a LOT of playtime), but I didn't get the Mega Drive in my room until Dad got an N64, and I didn't get Sonic 2 till the Mega Drive was in my room. So it's between F-22 Interceptor and pic related.

But this is the first game I truly devoted myself to. Rented it so many times from Blockbuster, but never beat it.
