To be comparable to the Max Headroom incident, it would have to be an event where the game experience was profoundly tampered with by a third party outside of the control of the publisher. This brings to mind various cases where people gained access to admin privileges in MMOs, or perhaps the flying dongs incident in Second Life (though you could argue in that case that everything was "working as intended" and Second Life isn't VIDEO GAMES anyway).
Can't really think of a case where this was done for non-multiplayer video games on a large scale. Perhaps you could gain access to some indie developer's web hosting and replace the binaries for their game with a modified version. Short of that, I guess you could go around to Redboxes replacing the disc in every copy of a certain game with some other disc as a prank.

grandparents live on a farm and have woods on the other side where grandpapy had a hunting tower set up.
grandma and aunt where sitting on porch talking and watching him come back when it was just getting dark. All of a sudden this big black object appeared over him that they said was about the size of a comercial jet. solid black, no noise or lights ovalish shaped.
They yelled for him to start running but he didn't hear them. the thing was over him till he steppes right in to the circle of light of a industrial light they had on the barn.
He had no idea what they where talking about and didn't hear or see a thing. over the next couple of days he got progressively sick. in about a week and a half he went to thr hospital in which the doctors said his body was just starting to shut down. They never said why other then "he was old" and that this shit happens. My aunt and grandma found it bull since my grandpas side has been known to live long and my grandpan specially was very active and watched his health his whole life.

seeing this shit reminds me that I live on the border of tornado alley, every spring we would get heavy storms and 3:4 times we'd get a big ass tornado that would come close enough to my neighborhood to have my family holding together in fear but it's almost like they're avoiding us somehow. We'd always follow the path of destruction the next day and it would always be curved like it's course had been changed and then corrected once our home was out of harms way
