If you enter LI from the centipede arena and the bug is still alive, Solaire gets the bad ending. To my knowledge, there are only three ways to avoid this and all of them suck- give 30 humanity to Fair Lady to open the LI shortcut, use a poison cloud pyromancy to kill the bug through the closed shortcut door, or don't talk to Solaire at all until you've found and ganked the bug.

Here's something I thought about while fighting O&S again yesterday:
If we kill Ornstein first, Smough smashes his body and gains electric powers. Not too out of the ordinary.
If we kill Smough first, Ornstein becomes fuckhueg but really stays mostly the same.
I always thought it was weird that Ornsteins "power up" was just becoming bigger, especially since Smough wasn't this 60ft colossus of a boss, just taller than Ornstein. The way I see it now is that if Ornstein in DS1 was an illusion like we're kinda guessing, then it makes sense; the increase in size is an increase in power from absorbing the remaining energy from Smough's corpse, and it's doing the whole "I've powered up and now I'm huge" thing.

Well first off Lords of Cinder =/= Lord Souls.
The very start of recorded history is Gwyn (and the other lord) taking the lord souls out of the first flame. Presumably that alone makes them linked to one another but at no point do the lord souls ever actually factor into interacting with the first flame beyond you fighting the guys who have them to prove you are tough.
Now grant you there should be a clear reason for the first flame existing but every universe needs its beginning and I'm willing to not ask the question of "what was before the age of ancients/where did the first flame come from" if the answer is just "maybe nothing who cares".
The age of ancients was pretty "nothing". Every bad and good thing from DaS comes from the opposite concepts that the flame gave us. Heat and cold, life and death, etc. So without the first flame everything is a tree, dragon, or mindless tree hollow zombie thing but at the same time that's not necessarily a bad thing.
The introduction of the flame brings basically every big concept to the world so it's both a good and bad thing I guess. In an ideal world the world would never turn to a zombie hellhole but it does so to say everyone would be better of just doing nothing rather than suffering isn't really wrong.

Souls don't go away forever you know, they get resurrected at the will of the fire. The bell of awakening pulled the lords of cinder back. Firekeeper and friends all come back to life because the fire brought them back.
Someone probably took gwyndolins soul back, it could have even been aldritch. Ornstein could have easily been brought back too. Gwyn gave him a super soul. Gwynevere probably resurrected him to protect her after the chosen undead's cycle ended. Who knows who brought him back for 3 but seeing as he went to slay the nameless king and king of storm it was likely the soul of cinder/gwyn.

It's said that a certain Goddess packed up shop after the undead curse hit and skipped town LONG ago, so I assume that many humans did the same, the problem being that the undead curse is hard to diagnose early on, and so in DS2 they talk about the undead hunts, implying that the curse is spreading throughout the world.
The game has the theme of cycles, where the events of the game happens with the MC, shit's fine for a while, and then the curse hits again hundreds or thousands of years later and they start all over again.

A VERY basic summary:
In the beginning the world was featureless, gray crags, arch trees and Dragon niggers. In the ground below, there was The Fire. A source of life energy. Hollows came to this Fire, and found powerful souls. They became Lords or Humans. Lords fought and killed the dragons.
Leaving the caves, they created Kingdoms on earth. Humanity flourishes.
However, the fire's power starts to dwindle. The Witch of Izaleth tries to create a new Flame. This fails spectacularily, and just becomes an engine for creating demons. Alarmed by this, Gwyn decides to use himself as fuel for the fire instead.
At the same time, a curse arises in humans. They get a Dark Sign on them. It turns them back into hollows, so they don't die. They turn back into the original creatures before the First Flame.
In DS1, many Kingdoms have collapsed. But many still remain. They send their undead to prisons in the north, guarded by Demons, so they do not cause problems in society.
When the flame from Gwyn's flame starts to die, an undead comes to collect souls to replace him and burn themselves. This starts a cycle of waning flames, and a new Lord of Cinder that continues until III.

I'm just gonna say it. I love Dark Souls in so many ways, but considering all the factors I just think Demon's Souls has a better world. Or at the very least better theming.
Every single thing in Dark Souls is dark as fuck. Everything ends tragically. I can't think of a single story that doesn't end in something awful unless you actively stop the story from progressing. And it's not like that's bad- plenty of great shit came out of it. Gough and the tree sap I especially like. Artorias and Sif. Et cetera.
But Demon's Souls had that sad shit too, and it meant more because there WAS glimmers of hope. Yes, Maiden Astrea and Garl Vinland sacrificed themselves for the wrong cause in vain. Yes, Ostrava died fighting his false father. And yes, Thoma's wife and daughter were slaughtered.
But you still build yourself a family that doesn't abandon you. You wish each other well and help the best you can. And in the end, you do truly defeat the great evil and save the land.

First there's just Archtrees and Everlasting Dragons and hollows. There is literally no concepts such as Life and Death or Light as Dark.
Until from nothingness (?) there begins Fire, which causes the forming of first Dark, where the original proto-humans came from. The most badasss or lucky of these find Lord Souls and receive great powers. A manlet named Furtive Pygmy also finds a special soul (Dark Soul), which becomes the core of mankind and is spread to everyone instead of being bound to one person. The other Great Lords kill Dragons and everything is fine for a moment, except for the fact that entropy or something else causes everything slowly go back to how it was.
So this Great Lord Gwyn lights himself up to stave off the entropy / dying of Fire. This becomes a loop in which there must be a new powerful being to relink the Fire (usually the player character). One of the effects of Fire dying out is the entire mankind slowly reverting back to its original form, i.e. mindless Hollows. People who get the Undead Curse / end up going Hollow are loathed because the collective majority decided this is kind of gross, and banishes them as far away as possible because due to their undying nature simply killing them doesn't work as a solution.

Entire kingdoms rising and falling, roughly at the same times as Fire is linked (everything is good again) or they're dying out (the latter is when the games take place). Probably thousands of years. Many of these kingdoms are actually built on the same spots as the old ones, hence some old areas are left relatively untouched, f.ex. Anor Londo which was Gwyndolin's personal palace.
