A big different is that Touhou is all made by one guy since making games is his hobby. A hobby that ended up making him really popular, but it's still a hobby nonetheless. It would be much less forgivable if an actual game company released a game with art like that.

The only time I've ever played Touhou was I think the 12th one a few years back and all I could think was "wow, this is a really fun game, I wish I didn't have to spent 30 fucking minutes skipping shit dialogue before each boss fight because poorly-drawn anime girls are making unfunny jokes"
That and the music always has a really catchy melody but the sounds used to make it are awful, which is why the remixes are always better than the actual soundtracks

Reason why people like ZUN Art is because of the character designs. Also because the style feels anime without feeling like the generic anime style you see today. The unique artstyle and silly but cute character designs are waht does it, regardless of ZUN's actual artistic ability.
