Follow the retard's guide. Most important step is renaming and replacing shit properly.
However, changing themes on sysNAND will kill menuhax, and by extension, if your NANDs are not unlinked yet then changing themes on either will kill it as well. Took me awhile to realize that. You also should be sure that you're installing menuhax in sysNAND, not emuNAND.

You are correct. But it is likely that, when you unlink NANDs, you will lose access to the eShop and to all the service that require the NNID validation (on the sysNAND, you lose them because of the lower version firmware. On the emuNAND, you lose it because it doesn't get correctly validated). That is solved by formatting the emuNAND too.
But if you unlink them and still can access it via emuNAND, then don't format it.
