Aside from Mal'Ganis actually being quite powerful and most of the Forsaken veterans being off in Northrend at the time I am pretty sure that it was more than just a "small" sect that rebelled against her rule. Most of the Forsaken members are quite edgy about being undead and getting treated like monsters by all others races, and on top of it their city is being ruled entirely by pre-war outsiders. Yes they had all of the Abomination guards and what ever other horrible stuff from the war storage, but still, an entire city of powerful characters and they just took it over by night.
Not to mention how Sylvanas got nervous about the dwindling population of her Forsaken post-Northrend invasion, despite having one long conflict in Lorderan and one expeditionary war in Outlands before without such thoughts.

since vanilla the point of the New Plague was to kill the living and the Scourge. I don't remember if the Forsaken are immune somehow or they just have the privilege of knowing when it's coming and having protective gear but I'm pretty sure it's the latter
