Our modern perceptions of "fat" are grossly (hah) distorted. A degree of "fatness" was indeed a sign of wealth and luxury, but the art-historical record suggests that "fat" would look more like this statue, supposedly of Charlemagne, which we might now call "slightly pudgy." Charlemagne, if the statue is an accurate representation of his body, would not have been particularly hindered by his weight. So Sam Tarly would indeed be an anomaly, and considered morbidly obese. And I can't help but noting that from a practical perspective, the relative cost of Sam keeping his weight would have been prohibitive; he should have shrunk down to a regular weight after joining the NW.
The most common mention of weight I see in the middle ages has to do with a person's holiness. The thinness of a monk denoted his austerity and dedication to his vows. Bernard of Clairvaux is described as silencing heretical critics by revealing his emaciated neck. Conversely, charges of fatness, and thus gluttony, were frequently leveled against those thought to have deviated from proper praxis. But I don't know of any examples where lay fatness was criticized by another layman.
It is a good general assumption that the nobility ate better than most, and this might make them appear to be "fat" (cf. Charlemagne) in comparison.

War of the Roses is unfortunately a topic I'm not as well versed in as I should be. I can, however, comment on The Hundred Years War which is fairly close chronologically.
Rape was certainly widespread in the Hundred Years War and was definitely seen as a problem. Armies, both French and English, wrought significant devastation on the areas they marched through both in the food they took and in the violent acts they committed. However, most of the time it was also widely condemned and periodically various commanders made a concentrated effort to prevent, although pretty much always with mixed success.
The French were obviously always encouraged to try and stop rape and plundering on the part of their soldiers. The King of France was obviously not supposed to be ransacking lands he technically ruled. The French Church, including the Avignon Papacy, was particularly keen to see this kind of thing stopped.
The English in contrast were pretty happy to sow destruction and discord for much of the war. While rape was not an explicit element of the Chevauchee tactic it certainly happened on those raids quit a bit. However, during the later reign of Henry V when it looked like he really could take the French crown he did a pretty abrupt about face on the issue. The Future King of France could hardly be seen to endorse violence against his rightful subjects and so Henry began efforts to limit the violence his own armies committed. Disobedient soldiers could be summarily executed for their behavior and while this doesn't seem to have stopped rape it seems to have limited it.

One thing you see a lot of in GoT (or at least the two books of it I read) that I haven't come across much in history is nobles raping people. While there is certainly an element of our sources probably white washing the behavior of their subjects it's worth remembering that the code of Chivalry was pretty explicit on how women were to be treated and there would have been a lot of social pressure to not violate that. Some nobles also brought their wives on campaign, which would be a further reason not to go about raping women. In a similar way, noble women were often (although not always) spared the fate of rape that peasant women would have suffered.
David Green's The Hundred Years War: A People's History has some great information on this. There's also a chapter on noncombatants in Medieval Warfare which was edited by Maurice Keen. I can't remember the author of that particular chapter right now though.

What were brothels really like in the medieval period? How accurate is the depiction of them in shows such as Game of Thrones? Were they ever as elaborate as they have been depicted in recent literature?
GoT is far from accurate. The houses were not as lavish, but the women working in brothels kept clean beds and linen. Much like modern brothels, they were seen as a necessary evil, and there were ordinances in many places that regulated them. Regulations concerning Prostitutes Dwelling in Brothels from the city of Nuremberg, dated around 1470 states the following:
"Also, the brothel keeper, man and woman, must provide the women living in their house with chambers, bed linens, and decent food, and they must feed them two meals a day and at every meal two decent dishes; and for such expenses each common woman living in the brothel must give the brothel keeper separately the sum of forty-two pence weekly, whether she uses the food or not. In addition the brothel keeper must make and hold a bath at least once a week in the house for the women living in the house, and this at his expense, not the women’s."

One aspect of medieval Europe that almost all "medieval fantasy" series gets wrong is cuisine. When we think of medieval feasting, images of huge lamb/pork chops and mountains of bread come to mind--- in other words, unsophisticated fare, cooked blandly. Nothing could be further from the truth. The food that would be served to kings and lords in medieval Europe was extremely complex. Most modern kitchens don't have the competence or equipment necessary to recreate what little of medieval cuisine we actually know about--- there's not a single restaurant in the world (to my knowledge) that serves dishes derived from 14th-century European cuisine.

Spices, spices, spices. Cooks were pretty intense about using spices in their dishes during the medieval period. In a late-14th century cookbook from France, the Menagier de Paris, more than 75% of dishes called for the use of spices such as pepper and grains of paradise. The later "blandness" of French haute-cuisine, in which the flavor of underlying ingredients is emphasized over that of spices, is a direct reaction against the spice-heavy cuisine of the Middle Ages.
Courses. In England, there were generally three courses during a meal, whereas in France, noblemen and royalty would frequently eat four or more courses during a meal--- a far cry from modern depictions which show fruits, meats and soups occupying the same table.
In short, medieval cuisine, at the highest levels, was highly ornate, incredibly complex and far more colorful than the food that you and I eat. Blues, greens and reds frequently appeared on the tables of the wealthy. Dishes were appreciated not only for their taste, but also for their aesthetic quality--- one might even argue that aesthetics trumped taste in the high cuisine of the late-14th century. Very different indeed from the mounds of mutton and beef we generally associate with food and dining in the Middle Ages.
