On my first viewing I thought he did it but subsequent viewings have changed my opinion and I now think he imagined the whole thing, although he definitely did hold a nailgun up to Jean's head in the apartment.

It doesn't matter if it was real or not. Bateman's psychopathic fantasies/killings were how we coped with living in a world of shallow conformity, a place where life had no meaning beyond what suit and tie you were wearing. At night he could escape from it into his fantasies of murder and sex, fueled by his obsessions with 80s pop culture and pornography.
But increasingly he had to do more to feel the same reprieve, until finally he broke down, realizing that no matter what happens, no matter what he does, he cannot escape, and his life will have no meaning.
