I worked in a restaurant in Manhattan for almost a year and never once saw a health inspector yet we still got passing grades. Strange how that works. The place was relatively okay, but some parts were just nasty as fuck. I took the initiative and scraped the overhead of the grills one day when we got closed down due to a generator blowing outside and I swear to god I scraped two inch thick sheets of rubberized grease off that shit that you could bend and it would then retain it's former shape. I bet no one had ever cleaned it before me and the place had been there at least a decade. The owner was Italian and drove a Lincoln Towncar with The Godfather theme as a horn, so you know, I kept my disgust to myself. He was good to me though, he'd come in and see me busting my ass and hand me extra money out of his pocket and a couple times he gave me bags of coke when I told him I was going out after work. Here in the UK we have sooo much overhead, red tape, laws, regulations, health safety through our local councils, local bodies, governments (scottish, welsh etc), country government (uk parliament) , EU and UN regulations.
