What's the worst character death you've seen in an RPG?

Define "worst". The most boring, the most painful, the most unusual, what?
I mean, one of my characters was stepped on by a golem, another was torn apart Hellraiser style by an inevitable's meat hooks. Another died quietly of old age in his sleep leaving the crown of an empire to his son and a grieving party since he was the only human and died long before any of the others were even balding.

Sifted by mooks after being paralyzed by a spell that, by every rule, should not have worked on me, especially by the one exempting my character literally in the same paragraph of the description, yet on the backside of the page.
Of course there was no takeback even though pretty much everyone died from this nonchalant fuck-up and there was more than a few miens showing a considerable chance of the GMs facial bones undergoing some restructuring in the near future.

In a GURPS campaign one of the players was being an absolute idiot while playing an alien that could be best described as a "10 foot sentient mantis shrimp" who was our weapons specialist. For some reason he decided it would be a great idea to go wandering around the space station we were at in an "anarchy" system with no formal government (I guess he translated that to mean literally anything goes) and, well, long story short, he was found by station security covered in blood with a hooker's half-charred dismembered legs in his backpack, and ended up getting shot with more holes than the plot of a Uwe Boll film.
My character, the ship's medic and engineer, a madman who had his medical license revoked for having a "difference of opinion" on where the line between organic and machinery should be drawn and is more or less a techpriest without the religion, gets his hands on the body by claiming his kind have some death rites and needs to be shipped home. I then, with a massive fortune made doing back alley surgery, spend about ten minutes listing all of the cybernetic and biologically engineered augmentations I'm adding to his character as I hack away at the corpse like a Need for Speed garage montage.
The result was something we ended up having to register as a "bodyguard model autonomous assault mech" instead of a crew member just to get through customs from then on. I even included a neural control system to give him a failsafe in case he did anything else as dumb as before, since now he had enough hardware in him to level a city block. I just left it off most of the time.
My favorite character got killed a few sessions ago. Got knocked off of a bridge into a extremely deep and dark chasm. It was a good three rounds of combat before the other players were notified of the SPLAT they heard as my barbarian pancaked on the chasm floor. They couldn't even retiree be my body and got so destructed they forgot to do even a single eulogy or false grave.

Definitely the most memorable one for me:
Our party just finished the long quest and was on the way-back to civilisation after tearing apart a manor belonging to vampiric baron. The campaign involved fighting entire covent of vampires and dealing with all-powerful magician. Party managed with all of that.
On the road magic-bound wolves we didn't dispose off on the way to the manor attacked.
Series of absolutely awful rolls later, all the horses run away or were killed, the priest was dead (GM was kind to allow re-roll in combat for wolf attack... it was even worse), the merc had badly injured calf and the bard had to play the doctor on everyone.
The merc died out of mistreatment of his wound, since nobody had any clue how to stop the bleeding on the long run.
The bard and the ranger then were on their own in the middle of nowhere. The bard was then mauled by a bear while trying to forage from random carcass in the forest.
The ranger, all alone, had a choice soon after - try to swim across the river, this time without horse's help, or go to the bridge. He should go for the bridge.

Animatrix. It's the scene where a piloted combat mech is held down by robotentacles while they cut their way into the torso/cockpit while the pilot screams 'help me help me'. Then they literally tear him out of it.

ok here's a story for you....
The party was in the astral sea and we were chasing after our ship, that was stolen by some dickhead mutineers, when the rouge came up with a really dumb idea (try as i might i couldn't convince the asshole this would DEFINITELY get him killed), we were going to shoot him out of the balista on the front of the dingy thing we were on.....i know......so we did, and well long story short he missed the ship by a long ways and careened in to a flock of these meat eating flying piranha things ( a bit like the krill from gears of war) and was eaten

But he just crawled right in.
