I work at a mental health facility, OP. These ARE red flags. The last two commonly affects everyone around them in the most negative way. The first one is an obvious sign of degeneracy and it's a no brainer why many of the clients who have dyed hair also have the last two. It's not surprising either that they have piercings other than ears (one of the ones I referred to drug rehab had those ear lobe things but on his face where it shows his teeth constantly) and tattoos.
