My theory is that Misaki is actually a hallucination, akin to something like a tulpa. This might seem absurd at first but consider the following evidence:
* Satou is prone to hallucinations throughout the entire series where he talks to fridges and becomes so delusional that he believes there's a conspiracy to ruin his life.
* In the manga Satou heavily abuses hallucinogenic drugs, contributing to an altered mental state which exacerbates existing mental illnesses.
What's interesting about Welcome To The NHK is the way it perfectly mirrors the authors life. In real life, Tatsuhiko Takimoto is a hopeless hikkiomori and its very likely that the book is based on fragments of his life. For example, he may have seen a girl knock on his door giving him religious pamphlets -- the first human contact he had seen in a long time. It's quite easy for him to start imagining that the girl is there to rescue him. Who wouldn't imagine that if there was no other hope? So he ends up writing an entire novel based on that theme.
In a way, Misaki is the author's hallucination and simultaneously Satou's. She is the perfect ideal of wish fulfillment, but I believe its much deeper than that. Satou and Tatsuhiko know that no one is going to come save them but they obviously need help to venture into the outside world. So along comes Misaki -- a perfectly engineered hallucination designed to do that. Misaki is the realization of hopelessness in human form. A complete hallucination when there's no other choice.
Tatsuhiko made up a fictional world where there was an answer and I believe Satou did the same. The moral of the story is that you have to help yourself any way you can and if delusion does that then I welcome it.
