The kind of surgeries that can cure uglyness (lefort fractures, sliding genioplasties) are extremely complicated and very few surgeons if any at all will perform them for cosmetic reasons. I've seen several maxfac surgeons this year trying to find one who will, with no success. I even got laughed out of a clinic once for suggesting it
