People who think gender and sex are dramatically different are people who have hijacked the meaning to fit their own social narrative and need a history lesson. Gender is used, and came up with because of a need to describe a larger biological basis for sexual differences in men and women(including sexual organs) other than the word sex because it has multiple meanings. It's almost interchangeable and is still used by both scientists and others to mean basically the same thing because it's applies to 99% of the population most of the time if you exclude things like Klinefelter's and Turner syndrome.
Transgender as thing is self is basically a myth because even with HRT you can't replace how the body on a cellular level uses things like estrogen if you're a male because of your different anatomy, and on a more advanced level progesterone and relaxin among other hormonal reactions. Other things like vaginoplasties are basically to put it bluntly glorified genital mutilation, for a instance mtf making epithelial lining in the vaginal walls, and that's getting into it, excluding the obvious like ovaries and a cervix.
Neurological is most more intricate, things like Hypothalamus and the Hippocampus are known to having different gender effects between men and women, which is not shown in any basis to be vastly similar to "trans" people. There is actually an interesting study on this on how transgender people revert back to their biological gender when certain drugs signifying is most likely indeed is a mental disorder.
