Technically you can have totally free healthcare but if you work a job, you pay a rather small amount of money and your employer pays a bit as well and that way the insurance company will take most or all of the costs for any necessary medical expenses.
You can buy better insurance that takes care of nearly any medical bill for you, like standard doc appointments and all that, but that costs a bit extra because it's not the bare minimum but kind of luxus-service. It's still not expensive for the average person since you only have to pay much when you have to pay many medical bills.
And if you are in a situation where you have to pay many bills, it's cheaper to pay the higher insurance rate than the entire bill yourself.
