Thats a charge slip. When I was waiting tables a lot of times people would pay the bill with a card and then leave the tip in cash. The reason for this is that if the tip is left on the card, then 100% of the tip has to be claimed as untaxed income. If the tip is left in cash, the server has to report a tip of at least 10% of the sale price as a tip for tax purposes. This means with cash tips, you can actually under-report your tipped income, which is a big advantage for the server come tax time.
It would not surprise me one bit if the tip was left in cash. Generally, people who have $187 to spend on a meal out have the extra $28.05 necessary for a 15% tip. Granted, the service could have been shit but then why would she deserve a tip anyway?
