My understanding from TV/movies, which can obviously be flawed, is there is a waiting list of sorts where all sorts of factors such as need and lifestyle (drug and alcohol use) are taken into consideration. They only get age of patient, current medical records, criminal record, and GPA (if relevant). Which they did for this individual.
