Well that loose tire thing can happen to anyone, nobody gets all the details right 100% of the time, though it is a pretty big fucking detail. Where you fucked up was not stopping sooner, imho.
Next time trust your instincts sooner and add tightening the lug nuts to your post maintenance check list.

I haven't had a notable fuckup yet aside from putting on tires without cleaning the caked on dirt and gravel on em, fucking up the balance.

Pumped the brakes while caliper was hinged outwards.
Pushed the piston out and broke the seal.
That was 2 years ago and I haven't fixed it.
