I just look back to "Leap of Faith" to consider how bad some ponies seem to have it. There were dozens of injured and horribly sick ponies willing to try snake oil to get healed. Either Equestria lacks the medical knowledge to care for them, or there's a pony poverty problem, and Ponyville and the Mane 6 are actually really high class elites with more money than the average pony.
I mean, there's AJ who runs a massive farm and has connections across Equestria.
